Vehicle loan default

Insurance claim

Ethel E. Lestishock, 46 Third St., Beaver Meadows, filed suit against Office of Federal Employees' Group Life Insurance, 5950 Airport Rd. Oriskany, N.Y.

The suit states that Lynn A. Petuch, daughter of the plaintiff, was insured by the defendant. The suit further states that on Jan. 28, 2009, Petuch suffered accidental death in Dauphin County and the plaintiff seeks the benefits of the policy held by her daughter. Sought is $112,000 and costs.
